FDA Approves Smaller Motiva Breast Implants for Subtle Results

The trend toward smaller, more natural-looking breast implants is getting another boost. The FDA has approved 28 additional sizes of Motiva breast implants, including options as small as 105 cc, giving plastic surgeons more flexibility to create subtle results for patients who want a little volume without a dramatic change in size.

The October 5 approval expands the available sizes of both Motiva SmoothSilk Round and SmoothSilk Ergonomix implants. It’s the latest development for the brand, which first received FDA approval in 2024 after years of use in international markets. The new sizes are expected to become available in the U.S. before the end of 2026.

The Shift Toward Smaller Implants

For years, breast augmentation was often associated with a noticeably fuller look. More recently, surgeons have been seeing growing interest in smaller implants that complement a patient’s existing shape rather than dramatically change it.

It’s a shift reflected in the growing conversation around natural-looking results, with some patients choosing smaller enhancements that feel more proportionate to their frames.

“Small-volume breast augmentation is a significant part of my practice, and many of my patients want a subtle enhancement that fits their frame rather than a dramatic change in size,” said New York plastic surgeon Anna Steve, MD, in the company’s announcement.

The expanded lineup includes 19 new sizes of SmoothSilk Round implants and nine new sizes of Ergonomix implants, with additional options across Mini, Demi and Full profiles. That variety matters because implant selection isn't just about volume. Width, projection, body proportions and existing breast tissue all play a role in how the final result looks.

What Makes Motiva Different?

When Motiva first entered the U.S. market, plastic surgeons were particularly interested in its SmoothSilk surface and soft, cohesive gel. The two implant styles also offer different aesthetic options: SmoothSilk Round maintains a rounder shape, while Ergonomix is designed to shift with body position for a more natural-looking contour.

Now, the smaller size range gives surgeons additional choices when working with patients who want a conservative enhancement or are considering a breast implant revision.

“With Motiva now available down to 105 cc and with more options in base and projection, I have a complete range to meet those needs,” Dr. Steve added.

The approval also fits into a broader evolution in breast surgery, including growing interest in less invasive approaches and techniques that prioritize natural proportions and tissue preservation.

For those considering an augmentation or implant swap, the takeaway is more choice. Smaller implants won’t necessarily be the right fit for everyone, and choosing a size still depends on anatomy, skin quality and aesthetic goals. But for patients who are considering a breast augmentation and want a subtle boost, the expanded range could make it easier to find the right fit.
